New Patient

 

As a new patient, it is our goal to establish a relationship with you. It is important to obtain background information such as your medical history. Please print out our new patient information packet and have it filled out prior to your visit to speed up the wait time in our office. We look forward to meeting you.

 

New patients with insurance:
Please bring your insurance card to your first appointment and be prepared to pay your copay at the time of service.

New patients without insurance:
Please be prepared to pay for your visit and any procedures at the time of service.

Insurance and Billing:
We accept all PPO insurances, Medicare, Healthy Families and Santa Barbara Select IPA HMO. We do not accept Medi-Cal, Sansum HMO or Physician’s Choice HMO. For Billing questions contact Melinda at (805)733-4869.

Payment Options:
We accept cash, check, and all major credit cards.
